I hereby direct GeorgeTutor to discontinue my monthly subscription tutoring services and close my account per the choices I made above.

I understand that my Notice to Terminate will become effective as soon as I have paid all fees due, including all arrears and late-payment fees (if any). and including payment in full for the billing period following the billing period in which my Notice to Terminate is given.

I understand that once my Notice to Terminate becomes effective, I will continue to receive service at the pre-planned delivery rate and according to the regular delivery policies up to the date which I have specified above for my student's last session.

I am aware of GeorgeTutor's policies regarding scheduling flexibility and understand that the flex tokens in my account are the property of GeorgeTutor and will be zeroed at the closure of my account.

I will endeavor to use or to give away any unexpired vouchers for GeorgeTutor service that remain in my account before the date of my student's last session.
