First, please note that our underlying Contract Agreement (made at registration) is unchanged by this slot allocation change request. However, we ask you please to confirm:

I hereby agree to accept the slot allocation change in my monthly tutoring subscription service as described by my choices above, provided that TVL is able to make the change.

I understand that the change in slot allocation may involve changing all my currently allocated slots at TVL's discretion.

I understand that a change of slot allocation may be assessed a fee of $150, if today's date is less than 4 months after my date of registration or my last slot allocation change, whichever is more recent.

I agree that if TVL is not able to make the change that I have requested, our existing Agreement and current slot allocation remain in effect. In particular, I agree that the Terms of Notice in the Agreement covering my existing subscription are not affected by the outcome of this slot allocation change request.
